Terms and Conditions – Admissions
Welcome to Fr333 Yourself Recovery Center (“the Center”). By accessing, submitting, or participating in our admissions process, you acknowledge that you have read, understood, and agreed to the following terms and conditions. These terms outline the expectations, responsibilities, and rights of individuals seeking admission into our mental health and substance use treatment programs.
1. Purpose of Admission
The purpose of the admissions process is to evaluate eligibility for entry into our clinical programs and to ensure that each client receives the appropriate level of care for their unique needs. Admission is not guaranteed and is subject to assessment outcomes, available space, and the professional judgment of our clinical team.
2. Confidentiality and Privacy
Fr333 Yourself Recovery Center adheres to all federal and state privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA). All personal, medical, and psychological information collected during the admissions process will remain confidential and will only be shared with authorized staff directly involved in your care, or as otherwise required by law. By submitting your application, you consent to the collection and use of your information for the purposes of evaluation, treatment planning, and service coordination.
3. Accuracy of Information
Applicants are required to provide complete and accurate information throughout the admissions process, including but not limited to medical history, substance use history, mental health background, and financial information. Providing false, misleading, or incomplete information may result in delays, denial of admission, or termination from the program if discovered after admission.
4. Clinical Assessment and Eligibility
All prospective clients must undergo a comprehensive clinical assessment to determine appropriateness for the level of care offered by the Center. Our treatment programs are designed for adults who can participate in structured residential care and follow treatment recommendations. Individuals who require medical detoxification or have conditions outside our scope of services may be referred to a higher level of care.
5. Financial Policies
Upon admission, clients or their legal representatives are responsible for understanding and completing all financial agreements. Fr333 Yourself Recovery Center accepts various forms of payment and insurance; however, coverage varies based on the individual’s policy and benefits. Any services not covered by insurance must be paid by the client. Financial arrangements must be confirmed prior to beginning treatment unless otherwise approved in writing. Failure to maintain financial responsibility may result in interruption or termination of services.
6. Program Expectations and Code of Conduct
Clients agree to follow all Center rules, policies, and treatment plans established by staff. Respectful behavior toward peers, staff, and property is mandatory. Possession or use of alcohol, illicit drugs, or unauthorized substances is strictly prohibited on the premises. Violent, threatening, or disruptive behavior may result in immediate discharge from the program.
7. Medical and Emergency Care
Fr333 Yourself Recovery Center provides behavioral health and recovery services but is not a medical hospital. We do not perform medical detoxification or provide inpatient medical services. In the event of a medical emergency, the client may be transferred to an appropriate facility for care. Clients agree to allow such emergency transfers when deemed necessary for their safety and well-being.
8. Treatment and Program Modification
Treatment plans are individualized and may include therapy, counseling, psychiatric services, holistic activities, and life skills training. Fr333 Yourself Recovery Center reserves the right to modify treatment plans, schedules, or program components at any time to enhance clinical outcomes, ensure safety, or meet licensing requirements.
9. Limitation of Liability
While our team strives to provide the highest quality of care, Fr333 Yourself Recovery Center cannot guarantee specific treatment outcomes. Recovery is a personal journey and depends on individual participation, motivation, and circumstances. By enrolling in our program, clients acknowledge and accept that the Center, its employees, and affiliates shall not be held liable for any specific results, relapses, or circumstances outside of reasonable control.
10. Consent to Communication
By submitting an application or inquiry, you authorize Fr333 Yourself Recovery Center to contact you by phone, email, or text for admissions updates, scheduling, or additional program information. Your contact information will not be shared or sold to any third parties.
11. Discharge and Completion
Completion of the program occurs when clinical goals are met as determined by the treatment team. Clients may also be discharged for non-compliance, medical necessity, or other reasons determined by staff. Aftercare planning will be provided to ensure continuity of support and transition to community-based services whenever possible.
12. Modifications to Terms
Fr333 Yourself Recovery Center reserves the right to update or modify these Terms and Conditions at any time without prior notice. The most current version will always be available on our website. Continued participation in the admissions process constitutes acceptance of any revisions.
13. Acknowledgment and Agreement
By submitting your admissions form, you acknowledge that you have read, understood, and agreed to these Terms and Conditions. You further affirm that all information provided is accurate and complete to the best of your knowledge and that you consent to participation in the admissions process under these terms.
